MYSA Forms

PLAYER FORMS (For Players to Complete)

MYSA Liability/Medical Release Form

Every player needs to complete this MYSA medical release form, and team managers should carry them to every league and tournament game. Note that the form only needs to be notarized if you will be traveling to tournaments outside of Minnesota.

TEAM FORMS (For Managers/Coaches to Complete)

Adult Background Check

This risk management form is now an online signup which all coaches, trainers, and managers MUST complete in order to receive a pass. Background checks will be conducted by McDowell Agency. The volunteer login is MYSVLY and the password is MYSVLY. Either print the 10-page instructions or open them in another window before you begin. You will need to refer to them in order to complete the process correctly (it should only take about 5 minutes).

MYSA Travel Permit (online signup or mail-in form)

You must register for a travel permit to play in any tournaments that are hosted by non-MYSA clubs. This would include all tournaments outside of Minnesota and the western border of Wisconsin (eg, Hudson, LaCrosse). Sign up at least 30 days prior to tournaments in North American or 60 days prior to international tournaments in order to pay the lowest fee.

MYSA Match Lineup Report

Your team must present this completed form to the head referee for every league game. Create a master by filling in the 'generic' information on the form and printing it. Then make copies so you can fill in the specific details for each game individually (date, time, field, opponent). The player registration number can be found on their player pass; all the game details will be on your final game schedule. IF YOU ARE THE HOME TEAM, you must also provide a stamped, addressed envelope to the referee with the form, so they can mail in the reports. The address to send it will be provided with the game schedule.

Ejected Participant Report

If a player is ejected from a game (ie, receives a red card), the referee will retain their player pass to send to MYSA and must complete the top portion of this form. The coach is responsible to have the referees of subsequent games sign this form until the required number of game suspensions has been served. Then fax it to the number indicated and call MYSA to arrange to pick up the pass.
